If you wish to cancel our contract, please fill out the following form
and confirm the shipment to us, SFM Hospital Products GmbH, by clicking the "Submit" button.
Your cancelation of contract will be sent to shop@sfm-berlin.de.
We will confirm the receipt of your revocation.
 
The email address must contain the email address used during the order process.
Processing with email addresses unrelated to the order is not possible.
 
If you wish to cancel your order on marketplaces, please do so on the respective marketplaces.
Processing via this form is not possible in these cases.
